I am hoping to identify the following keyboard (see images attached). Note that a few keys appear to be missing as well.

It is a rubber dome keyboard with no decoder circuitry attached.
The only markings are:
TT 2/2
JFK-4004-03

A small sticker on the backside with the number 58.08.241.
